Abstract
Introduction: Non-suicidal self-injury is a disorder that has recently increased among youth, creating significant costs for families and public health. This disorder is associated with negative outcomes, including increased suicide risk. Accordingly, this research aims to examine the effectiveness of Barlow's unified protocol transdiagnostic treatment on self-injury behavior, experiential avoidance and neuroticism in individuals with non-suicidal self-injury.
Method: This quasi-experimental study used a pretest-posttest design with a control and follow-up group. The statistical population comprised all women aged 18-27 who, in 1403, presented to counseling centers in Mashhad with non-suicidal self-injury. The sample included 30 participants, volunteer-selected and randomly assigned to experimental and control groups. The experimental group received 8 individual therapy sessions; the control group, treatment post-follow-up. Data, collected via the Inventory of Statements About Self-injury (ISAS), neuroticism subscale of Multidimensional Emotional Disorders Inventory (MEDI) , and Brief Experiential Avoidance Questionnaire ((BEAQ), were analyzed using repeated measures ANOVA in SPSS-26.
Results: Results showed that the unified protocol transdiagnostic significantly reduced the frequency and severity of non-suicidal self-injury (intrapersonal and interpersonal functions) in the experimental group. This treatment also decreased the experimental group's neuroticism and experiential avoidance (P = 0.05). These effects were maintained at follow-up.
Discussion and Conclusion: Findings suggest that Barlow's unified protocol transdiagnostic treatment, by increasing mindfulness and reducing avoidance, can effectively treat self-injury and its motivations. Therefore, this treatment is essential for reducing non-suicidal self-injury and related factors in the future.
