اختلالات روان‌پزشکی و بدکارکردی های جنسی

چکیده

بدکارکردی­­های جنسی، از شیوع بالایی در بیماران روان‌پزشکی برخوردار هستند. این بدکارکردی ­­ها، ممکن است به دلیل آسیب‌شناسی و یا رویکردهای دارودرمانی اختلالات روان‌پزشکی شکل بگیرند. مطالعات متعددی به بررسی رابطه بین بدکارکردی ­­های جنسی و اختلالات روان‌پزشکی پرداخته­­اند. در این مطالعه مروری سعی شده است با بررسی پژوهش‌های متعدد انجام شده در این حوزه، این رابطه و ابعاد آن بررسی گردند.  به طور کلی، نتایج مطالعات نشان دهنده نقش موثر اختلالات روان‌پزشکی و داروهای مصرفی در شکل­­ گیری بدکارکردی ­­های جنسی می‌باشند. علایم مثبت اسکیزوفرنی (مانند سایکوز و توهمات) و علائم منفی آن (مانند فقدان احساس لذت)، ممکن است به شکل منفی باعث اختلال در روابط بین‌فردی و روابط جنسی گردند. افسردگی و اختلالات خلقی ممکن است بر میل جنسی، برانگیختگی جنسی، ارگاسم و عملکرد نعوظ تأثیر بگذارند. داروهای ضدافسردگی­ کلاسیک نسبت به ضدافسردگی­­ های نسل دوم، اثرات جانبی بیشتری بر کارکردهای جنسی دارند. همچنین، اختلالات اضطرابی بر بدکارکردی های جنسی مانند اختلال میل جنسی، عملکرد نعوظ و مشکلات ارگاسم تأثیر می گذارند. بیماران دارای اختلالات خوردن، ممکن است از مشکلات جنسی رنج ببرند. اختلالات شخصیت، به‌ویژه اختلال شخصیت مرزی و اختلال شخصیت نمایشی با بدکارکردی­­ های جنسی ارتباط دارند. علیرغم تمامی این نتایج، مطالعه در این حوزه دارای محدودیت‌های روش‌شناسی اساسی می‌باشد که باید در مطالعات آتی، مورد توجه قرار گیرد.

Sexual dysfunctions have a high prevalence in psychiatric patients. These dysfunctions may arise due to the pathology or drug treatment approaches of psychiatric disorders. Several studies have explored the connection between sexual dysfunctions and psychiatric disorders. This review study aims to investigate this relationship and its various aspects by analyzing numerous research studies in this area. Overall, the findings of these studies indicate the significant impact of psychiatric disorders and medications on the development of sexual dysfunctions. Positive symptoms of schizophrenia (such as psychosis and hallucinations) and its negative symptoms (such as anhedonia) can have adverse effects on interpersonal and sexual relationships. Depression and mood disorders can influence libido, sexual arousal, orgasm, and erectile function. Traditional antidepressants have more adverse effects on sexual functions compared to second-generation antidepressants. Additionally, anxiety disorders can impact sexual dysfunctions like sexual desire disorder, erectile function, and orgasm issues. Individuals with feeding and eating disorders may experience sexual problems. Personality disorders, particularly borderline personality disorder and histrionic personality disorder, are associated with sexual dysfunctions. Despite these findings, studies in this field have fundamental methodological limitations that need to be considered in future research.
